Pancreatic ductal adenocarcinoma (PDAC) remains one of the deadliest malignancies, with limited curative options. The only potential cure is surgical resection with negative (R0) margins. Despite advances in preoperative (neoadjuvant) therapy, high rates of positive margins persist. Building upon our prior bi-institutional study demonstrating that delta radiomic features (DRFs) from pre- and post-treatment CT scans predict resection margin status and survival (Wang et al., Ann Surg Oncol, 2023), this proposal aims to prospectively validate and expand these findings using a multimodal dataset integrating imaging, ctDNA, and clinical data. Our broad objective is to develop a predictive model for surgical outcomes, guiding personalized treatment strategies for PDAC.
