abstract

  • Purpose: The purpose of this project is build a co-location infrastructure and implement an integrated care model at the ECU Brody School of Medicines Family Medicine Center. Objectives: 1. Improving efficiencies in the identification, assessment and care planning processes. 2. Create individualized care plans and link patients, when appropriate, with existing disease and pharmacy management initiates 3. Reduce rate of ED visits and inpatient utilization 4. Improve satisfaction for both providers and patients Method: Newly established patients, patients with chronic disease, and patients being assessed and treated for a mood or behavioral disorder will be identified as the target population for this project. The PHQ 9 will be administered as a screening tool for depression among adult patients who are in this target population. Primary care and behavioral health providers will be administered a pre and post test survey to measure change in their attitudes and utilizations of integrated care models. All data will be sent to Access East as part of their evaluation and reporting requirements.
