Since 2006, a partnership was formed between East Carolina University?s Marriage and Family Therapy (MFT) masters and Medical Family Therapy doctoral programs along with Greene County Healthcare, Inc to provide integrated behavioral healthcare to GCHC patient. This grant will strengthen existing integrated care systems in the following ways: (1) GCHC will increase the number of people who can access mental and behavioral health by providing increased IC coverage by MedFTs at each GCHC site and (2) GCHC will increase options for care by including systemically trained master?s students to serve as health coaches at each GCHC site. Coaches will assess for common BH concerns (e.g. challenges with sleep hygiene, stress, nutrition, and activity levels), construct health goals based on patient?s readiness for change, collaborate with relevant healthcare providers to increase the likelihood for change, and follow up with patients to track progress. MedFTs will focus specifically on patients who have mental health and/or substance dependence symptoms or diagnoses. MedFTs will assess patients? symptoms against the new DSM-5 criteria and confirm accurate diagnoses. MedFTs will collaborate with primary care providers to ensure that indicated treatment is coordinated and delivered in accordance with a Level 5 IC model.
